This Agreement is entered into by and between: Naomi Silverman, NLP MASTER, Transformation Guide - (Guide)
and ____ (Client)
whereby Guide agrees to provide Transformation & Growth Services for Client
DESCRIPTION OF TRANSFORMATION & GROWTH:
Transformation & Growth is a partnership (defined as an alliance, not a legal business partnership) between the Guide and the Client in a creative process that inspires the client to maximize personal and professional potential. It is designed to facilitate the creation/development of personal, professional or lifestyle goals and to develop and carry out a strategy/plan for achieving those goals.
DISCLAIMER
Naomi Silverman [The Guide] (herein referred to as "Guide) is not an employee, agent, lawyer, doctor, manager, therapist, public relations or business manager, registered dietician, or financial analyst, psychotherapist or accountant. Client understands that Guide has not promised, shall not be obligated to and will nor (1) procure or attempt to procure employment or business or sales for Client: (2) Perform any business management functions including but not limited to, accounting, tax or investment consulting, or advice with regard thereto: (3) act as a therapist providing psychoanalysis, psychological counseling or behavioral therapy; (4) act as a public relations manager (5) act as a publicist to procure any publicity, interviews, write-ups, features, television, pant or digital media exposure for Client: (6) introduce Client to Consultant's full network of contacts, media partners or business partners. Client understands that a relationship does not exist between the parties after the conclusion of this program. If the Parties continue their relationship, a separate agreement will be entered into.
PAYMENT
Payment is to be paid in full 48 hours before the session. PayPal is best for international clients. Bank Transfer or Bit for local clients
CANCELLATION POLICY
Client agrees that it is the Client's responsibility to notify the Guide 24 hours in advance of the scheduled calls/meetings. Guide reserves the right to bill Client for a missed meeting. Guide will attempt in good faith to reschedule the missed meeting.
GUIDE -CLIENT RELATIONSHIP
A. Guide agrees to maintain the ethics and standards of behavior established by the ANLP.
B. Client is solely responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the Guide relationship and his/her calls and interactions with the Guide. As such, the Client agrees that the Guide is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Guide. Client understands “Transformation & Growth ” is not psychotherapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.
C. Client further acknowledges that he/she may terminate or discontinue the guiding relationship at any time.
D. Client acknowledges that Transformation & Growth is a comprehensive process that may involve different areas of his or her life, including work, finances, health, relationships, education and recreation. The Client agrees that deciding how to handle these issues, incorporate NLP principles into those areas and implementing choices is exclusively the Client’s responsibility.
E. Client acknowledges that Transformation & Growth does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that it is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the working relationship agreed upon by the Client and the Guide
.F. The Client understands that in order to enhance the relationship, the Client agrees to communicate honestly, be open to feedback and assistance and to create the time and energy to participate fully in the program.
MODALITIES USED:
Guide may use any or all of the following modalities unless client requests otherwise: NLP, EFT Tapping, Breathing Exercises, Guided Imagery, Energy Healing, OldPain2Go. Guide will be happy to answer questions about all the modalities. Client acknowledges that Guide’s certificates are in NLP and Energy Healing and has certificate of attendance in EFT Tapping and that Guide’s academic degree is in computer science.
